产品描述	Dofetilide (UK 68789) is a sulfonamide class III antiarrhythmic agent and potassium channel blocker. Dofetilide selectively blocks cardiac ion channels of the rapid component of the delayed rectifier potassium current Ikr. This antiarrhythmic agent prolongs cardiac action potential duration and effective refractory period due to delayed repolarization without affecting conduction velocity. This results in a normal sinus rhythm. Dofetilide is used in the treatment of atrial fibrillation and flutter.
体外活性	Dofetilide在Xenopus卵母细胞的切除巨贴片中阻断HERG电流。[1] 在豚鼠心肌细胞中，Dofetilide（1 μM）通过200毫秒测试脉冲和分析IKr去极化尾电流，将IKr的幅度降至控制电流的61%。[2] Dofetilide在完整的犬心脏中增加顶底复极化的差异，这是由于顶部ERP的增加比基部更为显著。[3]
体内活性	Dofetilide（100 mg/kg，静脉注射）不抑制由两阶段冠状动脉结扎和肾上腺素或冠状动脉结扎和再灌注引起的自动性心律失常，但能抑制在老年心肌梗塞（MI）犬只中由PES引发的重入性心律失常。Dofetilide在一些患有洋地黄心律失常的犬只中也显示出抗心律失常效应。Dofetilide延长QT间期，并表现出与其他III类化合物相似的负性频率效应，但在抗心律失常剖面上与其他III类化合物例如D-sotalol、E-4031和MS-551不同，因为它不能防止冠状动脉再灌注后立即出现的心室颤动（VF），并且对洋地黄心律失常有一定的抗心律失常效果。[4] Dofetilide在斯普拉格-道利（Sprague-Dawley）大鼠中增加了胚胎吸收并导致同一阶段依赖的畸形。[5]
